Requirements

• Active Illinois license or IMLC (LOQ) on hand.
• Board Certified or Eligible within 5 years in Family or Emergency Medicine.
• Must provide documentation of 50 walk-in clinic patients treated in the last year.
• Minimum of 24 months of clinical experience required.
